Getting Ready for Your Visit

Please plan to arrive 30 minutes prior to your scheduled appointment time to complete the paperwork required by your insurance company.

In order for us to provide the best care, it is very important you bring the following items to your visit:

• Photo ID

• Insurance information and card

• A list of your current medications

• Previous X-ray or MRI films with reports from radiologists

• Information about what treatments and medications have been tried in the past

• Medical records from prior visits to other healthcare providers

• New patients can download a patient registration packet here
